Phone Consultations for Mail Orders

Your phone consultations appointment will take 25 to 35 minutes to assist you with your skin care concerns, lifestyle adjustments and to review treatment products you will be using at home.

Please fill out the Client Informed Consultation Form below completely and as specifically & accurately as possible and email or fax us the forms. This will help us address your skin care conditions, and help you address your skin care needs. We also address many lifestyle issues that can affect your skin. We will make every effort to assist you as much as possible.

Skin Care Mail Order Packages for initial consultations are a prepaid fee of $55.00 and will be applied toward skin care packages if purchased the day of your phone consultation. If no purchase is made there is a prepaid $55.00 fee for the consultation. We offer a full line of pharmaceutical grade result – oriented therapeutic products for your face & body.

For best results use all products exactly as directed -it is also suggested that you get refills when your product/s become half full so as not to run out of your products. This is so that you have a consistent protocol to keep the skin normalized and balanced and in optimum health. Refills are not included in the package and must be purchased at separate regular retail prices along with applicable sales tax, shipping and handling fees.

Payments: We accept all major credit cards, cash (in the clinic only) postal money orders, personal checks with proper I.D. and on- line secure Pay Pal payments. Your shipping address must match your credit card address and some one must be available during the day to receive and sign for your package.

Shipping: We ship FEDEX Ground, 1Day, 2Day, 3Day and Overnight Express and UPS Ground,. We will process and package your orders ASAP. All checks must clear first before any products are shipped. Please allow 7-14 business days for delivery.

Exchanges: There will only be an exchange for a problem product/s if there is a true skin allergy (swelling, severe redness and rash) during the first 7 days after you receive your products. No refunds and no returns or exchanges after 7 days. No exceptions. No substitutes.
